Does an accident report have to be filled out at the time of the fall?

Ideally, an accident report or incident report should be completed at the time of your slip and fall. This should include relevant information and contact information for employees, as well as witnesses. However, there is no requirement that an accident report or incident report be completed for you to pursue a claim following your slip and fall.
